S: PT: presents for appt 3 on 6 Upper & lower Implants surgery With DR: Roberts Rmh,,nsf, ioe/wnl. pt;signed consent forms, took pano x-ray admin 1 tab ibuprofen 800mg had pt rinse w/ chlorhexidine mouth rinse Nitrous Oxide- 65 65 was given to PT admin 14 carpules 2 lidocaine w/1:100,000 epi opened envelope flap simply removed all U/L Teeth drilled pilot holes ,5mm placed direction indicating pins and took pa expanded holes using nobel replace tapered system HI-Tec implant used torque wrench to finalize placementWe added 2 more Implants on top into # 8 ,& 9 -To do 4 singe crown on anterior instead of bridge DR: Roberts :told PT: that it will be additional $ 2000 more to add 2 more implants on top and he was okay with placed bone graft material ,mineralized ,cortical / cancellous , cadaver bone Took pa implants placed nicely ,placed 25 on U/L simple suture rinse with / chlorhexidine 0.12 mouth rinse RX:amoxicillin : 500 mg disp 21 take 1 tab 3 times aday for 1 week ibuprofen :800 mg disp 15 take 1 tab 6-8 hrs as needed for pain Norco 7.5 mg Disp 15 take 4-6hrs as needed for pain Chlorhexidine oral rinse 0.12 rinse twice a day for 2 weeks Blaine ADJ And DeliveredUpper & Lower Dentures NV: 1- Week for post up Assisted by : Nermina
